Student Health Records

A health record, which includes medical history, physical examinations, screening results, and emergency information, is kept on each student throughout his/her school career. Please forward all medical updates, i.e. immunizations, medications, physical exams, and changes in health status to your school nurse.

In accordance with Massachusetts General Law CMR 23.06 (3), all student health records will be destroyed within five years of the date that a student withdraws from the Stoughton Public School System. The only exception is that the health record of any student who received an immunization in the Stoughton Public School System will be kept for at least 10 years following the end of the calendar year in which the vaccination was received.

Each student who is scheduled for graduation will be given his/her health record in a sealed envelope marked "Confidential Health Record." Each student will sign a form acknowledging receipt of the health record. A copy of each graduate's immunization record will be kept in a locked cabinet in the Nursing Director's Office and marked for destruction within five years of graduation, unless the immunizations were given in the Stoughton Public Schools and then the record will be kept for at least ten years following the end of the calendar year in which the vaccination was given.
